The client is an established German healthcare distributor with deep domain expertise and a clear thesis: telehealth would unlock a new patient channel for them within 12 months — but only if they could ship a compliant, production-grade clinic before competitors caught up.
What they had: capital, regulatory know-how, pharmacy partnerships, doctors. What they didn’t have: a product team, an engineering team, or the appetite to spend a year hiring one.

We started with a 4-hour Discovery Workshop to ruthlessly prioritize. The MVP cut: appointment booking, video consultations, electronic prescriptions, pharmacy fulfillment. Everything else moved to phase two.
Two engineers and one designer ran in parallel to the client’s business team. No project managers, no status meetings — just a weekly Friday demo of working software in a live staging environment, and a 48-hour resolution window if anything went sideways.

Backend on Kotlin and Spring Boot. Frontend on Next.js. Postgres for data. Infrastructure on AWS, EU regions only, with separate environments for staging and production. Every choice was a known quantity — nothing trendy, nothing the client’s eventual in-house team couldn’t pick up.
Compliance was scoped in from day one: GDPR-compliant data handling, qualified electronic signatures for prescriptions per eIDAS, encrypted video consultations, role-based access control with audit trails.
Launched on time. On budget. Now serving hundreds.
Day 90: production deployment, full handover, complete documentation. The client owns 100% of the code. We were retained for a focused 30-day stabilization phase, then stepped away cleanly — exactly as designed.
The clinic now serves hundreds of patients across Germany. The client has since hired their own engineering team to extend the platform — using the architecture and documentation we left behind as the starting point.
